RESEARCH HIGHLIGHTS Members of the Division have been reasonably successful in establishing research collaborations with the Division of Radiation Oncology. Three themes are extremely active at the moment.

| Faslodex alcoholAre less efficacious at inhibiting the aromatase enzyme. This is reflected in the fact that compared with the transplanted tumors treated with letrozole, E2 concentrations in the tumors treated with anastrozole and formestane are 1.5-fold and 2.5-fold higher, respectively. Although serum E2 levels were lower in the animals treated with anastrozole and formestane compared with the animals treated with letrozole, our results suggest that tissue E2 concentrations may be more important, because this is a reflection of the amount of E2 available in the tumor microenvironment. The half-life of anastrozole in rodents has been reported to be considerably shorter than the half-life of letrozole 47 ; . Therefore, the circulating levels of anastrozole may not approach those of letrozole. Thus, anastrozole may not be completely inhibiting tumor aromatase activity and, as a consequence, tumor growth. However, it is important to note that in our previous studies 30, 31 ; we reported that anastrozole had potent antitumor activity at a daily dose of 10 g day, and in this study anastrozole was administered at ten times that dose. After transplantation of the long-term letrozole-treated tumors, letrozole slowed but did not regress tumor growth, and tumor volumes actually increased slowly over the duration of the experiment. Thus, the transplanted tumors appear to have acquired at least partial but not complete resistance to letrozole. This suggests that there is a possibility of returning to letrozole therapy after initial relapse and second-line therapy with an antiestrogen. Nonetheless, in these experiments the transplanted long-term letrozole-treated tumors retained sensitivity to secondline treatment with the antiestrogens tamoxifen and faslodex. The pure antiestrogen faslodex was very effective at slowing tumor growth. In fact, over the 8-week duration of the experiment the volumes of the tumors treated with faslodex increased by only 0.55-fold. In summary, we have used the aromatase-transfected, estrogendependent MCF-7Ca human breast cancer cell line, and shown that when cells and tumor xenografts acquire the ability to proliferate in an estrogen-depleted environment in vitro they remain sensitive to second-line therapy with tamoxifen. Moreover, after a long-term treatment with the aromatase inhibitor letrozole in vivo, transplanted MCF-7Ca tumor xenografts are sensitive in the second-line to tamoxifen and to a greater extent to faslodex. This is most likely because of the increased cellular ER levels that enhance the sensitivity of estrogen-deprived cells to the antiproliferative effects of antiestrogens and felbamate.

In addition, estradiol administration significantly reduced the incidence and duration of reperfusion-induced ventricular tachycardia and ventricular fibrillation. Although 5-hydroxydecanoate alone caused no significant effect on the incidence of reperfusion arrhythmias in the presence or absence of estradiol, the administration of HMR 1098 abolished estrogen-induced improvement of reperfusion arrhythmias. Pretreatment with the estrogen-receptor antagonist faslodex ICI 182, 780 ; did not alter estrogen-induced infarct-limiting and antiarrhythmic effects. These results demonstrate that estrogen is cardioprotective against infarct sizes and fatal reperfusion arrhythmias by different ATP-sensitive K channels for an estrogen receptor-independent mechanism. The infarct size-limiting and antiarrhythmic effects of estrogen were abolished by 5-hydroxydecanoate and HMR 1098, suggesting that the effects may result from activation of the mitochondrial and sarcolemmal ATP-sensitive K channels, respectively and fennel.

With his specialty training, he had done much work with the optic nerve and had a special interest in learning more about glaucoma. During his visit Sanke accompanied faculty members as they saw patients and performed surgery, and he attended education programs such as grand rounds and research seminars. Details of the Incident: On September 30, 2000, the second day of the Intifada a crowd of Palestinian policemen and civilians, some armed, attacked repeatedly an isolated Israel Defence Force IDF ; outpost situated at a road junction near Netzarim, an Israeli village in Gaza. French and other television groups televised the incident. The event developed initially as a demonstration and turned violent when Palestinian policemen began to shoot at IDF soldiers in the outpost. French television showed a father and his son Muhammed al-Dura hiding behind a concrete barrel. The 12-year-old boy seemed to be killed and his father seemed to be wounded. The French television alleged that A-Dura was killed by IDF fire. The IDF soldiers claimed that they did not see the pair crouched behind the barrel and they didn't shoot at them. The pictures of the event were shown all over the world and Muhammad a-Dura became the symbol of the Palestinian intifada. Methodology: No autopsy examination of the body was made by the Palestinians, there are no accurate details of any wounds that may have been caused by the gunfire. The body was not identified according to accepted standards and recognized practices. The identity of the body cannot therefore be accepted as that of Mohammed a-Dura the boy A-Dura was clearly not killed by IDF fire. Aerial photographs were taken of the scene that showed the crossroad and the position of the Israeli Defence Force IDF ; outpost. Television footage of scenes of the riot, the boy cowering with his father and of the funeral procession were examined as well as medical notes and still photographs of the boy lying on a mortuary slab and filgrastim.

Ject to greater risks than those in other groups if they are prescribed an "equivalent" drug because substantial evidence indicates that, even within a class, drug effectiveness and toxicity can vary among racial and ethnic groups. Institutional drug formularies and step-care protocols should be broad enough to allow rational choices of drugs and dosages for all patients, regardless of race or ethnic origin. Pharmaceutical companies should continue to include significant numbers of patients from varied racial and ethnic backgrounds in drug metabolism and clinical trials in instances where genetic polymorphism for that drug class is relevant. The vast majority of drug manufacturers test and evaluate new pharmacological compounds on population subgroups, including racial and ethnic subgroups. This is likely to reveal drug actions and side effects specific to these groups, and may lead to the discovery of therapies of specific advantage to these populations. Health care providers should give individualized treatment to each patient and resist the temptation to apply "cookbook" drug therapy that does not take into account racial or ethnic origin. For the practicing physician, each patient represents a unique and dynamic interaction among determinants that are both genetic and environmental. While it may be impossible for a physician to anticipate how a particular patient will respond in every instance, it is imperative to individualize therapy with respect to the appropriate choice of both drug and dose. Physicians should be alert to atypical drug responses or unexpected side effects when they treat patients from varied racial and ethnic backgrounds. Dosage adjustments should be made for patients from different racial and ethnic groups if pharmacological evidence supports the adjustment. Finally, and perhaps most important, all and felbamate.
