The Chinese name for epimedium herb yin yang huo, or horny goat weed ; derives from folklore accounts that originated in the northern plains of China. It seems that goats in this region that grazed on this weed would--how can I say this gently--experience increased emissions spilling onto the grass. Farmers tend to notice this sort of thing. Recently, I've heard rumors that this herb is gaining increasing popularity in Hollywood, as one might expect. We use it as a frequent addition to formulas for treating impotence, low sex drive and frequent urination. It works for both men and women. It is also useful for hypothyroid conditions. The concentrated powder can be dissolved in wine. Research highlights Pharmacological and animal studies in China indicate that epimedium increases sperm production, increases sexual desire, and stimulates the sensory nerves. It also increases mating behavior in animals reported in Yeung, 1983; reported in Bensky and Gamble, 1993; Dong et al., 1994; Kuang et al., 1989 ; . Epimedium extracts polysaccharides and glycosides ; stimulate the immune system and have shown the ability to reverse suppressed immunity in animal models and clinical trials on humans. The glycosides increase coronary flow, reduce blood pressure slightly, and exhibit liver-protective effects reported in Huang, 1999, Lee et al., 1995 ; . In a study of rats with kidney disease, epimedium reduced the level of BUN and serum creatinine Cheng et al., 1994 ; . Epimedium decoction proved very effective in a controlled trial on patients with chronic kidney failure who required hemodialysis. The herb increased their sexual drive significantly and improved their immune function, as well as the overall quality of the patients' lives Liao, 1995 ; . Researchers have tested epimedium in vitro and in vivo for its effects on osteoporosis. In the test tube, it improves bone resorption by osteoclasts ; , and in rats it increases mineral content and promotes bone formation Yu et al., 1999 ; . In a study on rats, water extract of epimedium reversed the side effects of long-term use of steroids, reducing adrenal atrophy and bone loss Wu et al., 1996!
Tively controlled when SAL was used with low-dose ICSs compared with higher doses of ICSs alone. Greater reductions in symptom scores and daily rescue use of albuterol were noted when SAL was used with ICSs compared with increasing the dose of ICS dose alone P .05 ; .30 A study by Sue-Chu and colleagues31 provides further support for the complementary effects of ICS and LABAs on controlling airway inflammation in patients with asthma. Fifty-six patients who were symptomatic despite ICS therapy were assigned at random to receive FP, 200 g twice daily n 19 FP, 500 g twice daily n 19 or FP, 200 g twice daily with SAL, 50 g twice daily n 18 ; for 12 weeks. Bronchial biopsies and methacholine challenges were done at baseline and after 12 weeks of treatment. In this study, the numbers of eosinophils were low in all groups at baseline and did not change significantly with treatment. However, the numbers of mast cells were reduced with FP, 200 g, plus SAL, 50 g, but not with FP alone at either the 200- g or 500- g dosages. Likewise, using SAL, 50 g, with FP, 200 g, was accompanied by decreases in CD3-, CD4-, and CD25positive lymphocytes that were similar to or greater than those seen with higher doses of FP alone.31 Finally, Kips and coworkers32 evaluated the effects of adding formoterol to low doses of budesonide compared with higher doses of budesonide alone on inflammatory markers eosinophils and eosinophil cationic protein ; in induced sputum. In this year-long study, patients received budesonide, 100 g, with formoterol, 12 g given twice daily n 29 ; , or budesonide, 400 g twice daily alone n 31 ; . The regimen of low-dose budesonide with formoterol controlled airway inflammation as assessed by sputum eosinophil numbers ; as well as a fourfold higher dose of budesonide.32.

SCE is similar to conventional EGD in many technical aspects, but there are also important differences. First, the small-caliber endoscope uses a design that differs slightly from its conventional counterpart. Second, unsedated SCE requires nasal, rather than oral, intubation. An understanding of these subtleties is important to maximize patient comfort and diagnostic accuracy.
Formoterol salmeterol
Tuberculosis is an increasingly important public health problem in developing countries. We studied 111 tuberculosis patients confirmed by mycobacteria isolation between 1998 and 2000 in Rio Grande, in south Brazil. One-hundred-thirteen pulmonary and extrapulmonary clinical materials were examined through culture by the Ogawa-Kudoh method and through smear examination by the Ziehl-Nielsen or Kinyoun methods. The identification of Mycobacterium tuberculosis was done through the usual phenotypical methods. The proportion method indirect technique ; was used to determine the resistance of isolates. The man woman ratio was 2.6: 1, 75% were white, and the group had a mean age of 39.712.7. Risk factors were present in 38.7% 34 men and 9 women, p 0.2 ; , with a mean age of 35 p 0.002 infection by HIV was the most frequent 28.8% ; , followed by alcoholism 16.2% ; and injected drug use 15.3% ; . The frequency of M. tuberculosis isolates was 99.1%. Pulmonary disease occurred in 88.3% of the cases, extrapulmonary in 9.9% and 1.8% in both. There was an association between extrapulmonary tuberculosis and the presence of risk factors p 0.0001 ; . Resistance to isoniazid was found in 4% of the isolates and to isoniazid and rifampin in 2%, all being patients with some risk factors. The profile of tuberculosis in the population of this study followed the pattern described for developing regions of the world. Without hesitation I would acknowledge that combination products with fixed doses of long-acting b-adrenoceptor agonists and inhaled corticosteroids are an established therapeutic option for asthma in patients with moderate and severe disease. These drugs have changed conceptual views of asthma treatment and may have simplified asthma management, which is probably one of their greatest merits! There is a reasonable scientific basis for the use of combination therapy [1] that convinced us that the fixed combination of bronchodilators with inhaled steroids is the treatment of choice, at least for patients that are not sufficiently controlled with a short-acting b-agonist and an inhaled steroid. I still find it puzzling that the same doses of drugs given in one inhaler should have a greater effect in these patients compared to the single components at least in the first weeks of treatment ; , but that9s the data and all published studies irrespective of sponsorship come to the same conclusion. As this is the evidence, I happy to adopt this strategy. But chronic obstructive pulmonary disease COPD ; ? We have all witnessed the heated discussions around inhaled steroids in COPD and have seen and read the data that confirm that asthma and COPD are completely different diseases, clinically and biologically. I can see the role of longacting bronchodilators for the treatment of COPD, an issue that is already addressed in the updated Global Initiative for Chronic Obstructive Lung Disease GOLD ; guidelines [2], but the use of inhaled steroids and combination therapy as in asthma? Is this a "one size fits all" strategy that is driven by commercial interests? And, were we all wrong, does this mean we no longer need to differentiate between asthma and COPD since the treatment will be the same in the end? At this point the available evidence should probably be considered. In this issue of the European Respiratory Journal, P. Calverley and co-authors [3] yes, the TRISTAN P. Calverley [4] ; present the second large clinical study of 2003 that demonstrates that maintenance treatment with a fixed dose of an inhaled steroid budesonide ; and long-acting bagonists formoterol ; in COPD improves lung function, quality of life and delays the time to first exacerbation and that this effect is more pronounced with the combination therapy than in either component alone. One-thousand and twenty-two COPD patients with 629 or 62% completing the study ; with a mean forced expiratory volume in one second FEV1 ; of 36% predicted were included in this multicentre trial and were randomised to receive either budesonide or formoterol or the combination of both or placebo, with terbutaline as rescue medication. Randomisation followed a run-in period of 2 weeks during which patients were treated with 30 mg oral prednisolone and formoterol b.i.d., and terbutaline as rescue medication. Three varieties of cultivated Codonopsis are particularly favored: Tianshui Dang Shen from Tianshui District, Gansu Province ; , Luzhou Dang Shen from Shanxi Province ; and Wen Dang Shen from Gansu Province ; Tonic herbalists can easily recognize high quality Codonopsis. Larger roots are the best. They should be straight and clean, without signs of insect, mold or fungus contamination. It should be dry on the surface, yet flexible and moist when a piece is chewed. It should be a light tan. High quality Codonopsis is sweet tasting and pleasant. The sweetness in the taste develops after you have chewed on the root for several seconds. Once you've gotten into the tonic herbs a bit, good Codonopsis will taste very pleasant when eaten raw. Low grade Codonopsis is much less tasty and poor Codonopsis has almost no flavor. Because low grade Codonopsis will sell for less, the suppliers spend less time cleaning and preparing it, so it might still be quite dirty. Remember, if you eat an herb like Codonopsis raw, it is best to wash it first to get any remnants of China's good earth off it. Always get the best Codonopsis you can obtain. Poor quality Codonopsis is of little value to one's health while high quality Codonopsis is a major tonic herb. Codonopsis is one of the herbs now regarded to possess significant immunostimulating properties. It is rich in immune stimulating polysaccharides which are extremely valuable in helping the body fight off microbial invasion, especially in the early stages. These same polysaccharides have been shown to be useful in supporting the immune systems of people with cancer who are using the herbs in conjunction with conventional cancer therapies Fu Zheng Therapy ; .
