This is a case of a 47 year-old male who presented with a left lower eyelid mass. History of present illness started 9 months prior to consult when the patient noted a 0.5 cm x 0.5 cm mass on the left lower eyelid. Patient also experienced occasional tearing, burning sensation & eye redness, OS. No consult was done or medications taken. 7 months prior to consult, patient noted increase in the size of the mass to 1 cm cm. Patient consulted a private physician and was given unrecalled eyedrops and Cephalexin TID, which afforded no relief. Persistence of the left lower eyelid mass prompted consult at our institution. On examination, visual acuity was 20 on both eyes. A left lower lid mass, which measured 2.5 cm x 1.5 cm x 1.6 cm, was noted. Patient underwent incision biopsy, which revealed Non-Hodgkin's Lym phoma, B Cell Phenotype, small cell type. Patient was referred to Medical-Oncology Department for Systemic work-up. The 12th US state approved the use of medical cannabis last month when New Mexico, in a reversal, voted to approve a measure to legalize cannabis for medical purposes. By a narrow margin, the House passed a bill that will allow patients to seek approval from the state to use cannabis to alleviate the symptoms of several debilitating medical conditions approved by the state Department of Health. This would include cancer, glaucoma, multiple sclerosis, spinal cord injuries, epilepsy and AIDS. New Mexico's bill is now sent to Governor Bill Richardson who already said that he will approve the measure. Indeed, he was a force behind the bill. Under the legislation, the Department of Health would be required to create a system for distributing medical cannabis to patients by October 1, 2007. They would issue identification cards to patients and caregivers, and also license producers to grow cannabis. The specific aim of this study was to conduct detailed qualitative research on the nature of the process of heroin addiction, dependence and specifically recovery, and the potential role of intervention within this. The research was exploratory in nature, and it was anticipated that by obtaining detailed reports from recovering heroin dependants, important issues would emerge that would help to provide a clearer insight into individuals' experiences of heroin dependence and recovery. Although some heroin dependants appear to recover without any recourse to treatment intervention, it is clear that many heroin dependants engage in lengthy and extensive contact with treatment agencies, and since treatment intervention is likely to be of considerable importance, at least to a proportion of heroin dependants, it is important that its potential role within the process of recovery be examined. This sort of mixed design approach needs to be adopted in individual studies with participants from a wide range of treatment services and interventions, and further analyses then undertaken across the various studies treatment services interventions ; . This is especially true for the new topical steroids such as budesonide Pulmicort ; and fluticasone Flixotide ; , so that there is virtually no systemic effect from oral absorption. The clinical effect of inhaled beclomethasone Aldecin, Becotide, Clenil, Respocort ; at a mean dose of 400 ng daily is equivalent to 7.5mg prednisolone daily.10 but the systemic concentration of beclomethasone is much less than that of prednisolone, thus reducing side-effects. 2. Topical steroid is very active pharmacologically on the airway surface, whereas systemic steroid is not. Hence a very small dose of topical steroid can be used for the same clinical effect. Beclomethasone has been used in asthma for nearly 24 years. Enough long-term data have now been accumulated to demonstrate its safety. From these data, we conclude that plasma membrane present at the surface of the cell before 0.5 Hz stimulation remained at the surface after stimulation. Thus, internalized membrane was the same as that added during granule fusion. In contrast, stimulation at 15 Hz gave a very different result. Much of the dye-labeled plasma membrane was internalized during endocytic membrane uptake, indicating mixing of granule and plasma membrane during exocytosis. Thus, to this point basal stimulation compared with stress activation results in smaller and slowed amperometric spikes, size-dependent internalization of fluid phase markers, retention of protein core markers within endosomes and segregation of granule cell-surface membrane. Endosomes generated under basal firing undergo rapid recycling Classic studies from the Winkler Lingg et al., 1983; Patzak and Winkler, 1986 ; and Kirshner Phillips et al., 1983 ; groups trace granule membrane endocytosis and trafficking to the trans-Golgi network. Membrane components reappear in newly formed and releasable secretory granules in 6 h. However, a few studies point to an additional more rapid recycling route that takes just minutes to complete von Grafenstein and Knight, 1992; Elhamdani et al., 2001 ; . We tested the possibility that relatively intact granules retrieved under 0.5 Hz stimulation might also undergo a rapid recycling mechanism. We labeled granule membranes during the exo-endocytic cycle and tested for rereleasability within a short time window. We bathed cells in 800 nM FM1-43 and stimulated at 0.5 Hz. As expected, fluorescence of cells increased as unstained granule membrane became exposed to the extracellular dye-containing Ringer's Smith and Betz, 1996 ; Fig. 5Ai ; . After the first stimulus train, we washed the cells with dye-free Ringer's and allowed them to rest for 4 min. Persistent fluorescence represents FM1-43-stained endosomes and marks membrane that recently underwent exocytosis and endocytosis. The cells were then restimulated with the expectation that if the dyelabeled endosomes rapidly reentered the releasable granule population, their re-exocytosis should result in the loss of the recently internalized dye into the dye-free Ringer's. This was found to be the case; we measured an activity-dependent decrease in fluorescence signal during the second stimulus train. A representative data set is supplied in Figure 5A. We confirmed that these rapidly recycling granules were functional by measuring catecholamine release under the same conditions Fig. 5Aii ; . However, when we conducted the same staining and destaining protocol by dyeloading under 15 Hz stimulation, we were not able to evoke rerelease of internalized fluorescence with a second stimulus train proportional dye rerelease during the second stimulus trains was 0.73 0.10 for 0.5 Hz vs 0.07 0.12 at 15 Hz; n 6 and 5, respectively ; Fig. 5B ; . This result indicates that endosomes formed under basal firing differ from those formed under stress activation; they are competent to undergo rapid recycling.
