DRUG INTERACTION POTENTIAL The potential for interactions between macrolides and other agents metabolized via the CYP pathway is well recognized and may be due to the formation of intermediate nitrosoalkane complexes with CYP, which then inhibit the metabolism of other agents via this route. Although telithromycin is a competitive inhibitor of CYP, it does not form inhibitory complexes in vitro.86 Common drug-drug interactions reported with some macrolides in humans are therefore less likely to occur with telithromycin. Several clinical studies have investigated the potential for drug-drug interactions with telithromycin.84 These studies suggest that interactions are unlikely between telithromycin and oral contraceptives, 87 warfarin, 88 paroxetine a CYP2D6 substrate ; , ranitidine, or antacids containing aluminum or magnesium hydroxide. There were no notable changes in electrocardiographic parameters on coadminis.

HISTOPATHOLOGY The histopathological inflammatory reactions to M tuberculosis can be organized along an immunopathological spectrum, as can be done with leprosy. A sequence from nonnecrotic epithelioid cell granulomas with no acid-fast bacilli high-immune ; , through necrotic epithelioid granulomas with some acid-fast bacilli, to necrosis with abundant acid-fast bacilli low-immune ; can be arranged. Lupus vulgaris typifies the high-immune pole; tuberculosis cutis orificialis and acute miliary tuberculosis, the low-immune pole. A similar immunopathological spectrum has been devised for cutaneous tuberculosis, extending from lupus vulgaris toward scrofuloderma through tuberculosis verrucosa cutis.40 In the classic case, the hallmark of the histopathological diagnosis of cutaneous tuberculosis is the presence of tuberculous or tuberculoid granulomata. However, the diagnosis may be missed if one searches solely for the classic tuberculoid granulomata. 41 Seven additional patterns of inflammation have been described42: Classic tuberculoid granulomas, which consist of typical granulomas with Langhans'type giant cells. A peripheral cuff of inflammatory cells, predominantly lymphocytes, surrounds the giant cells. Caseation necrosis may or may not be present Figure 15-1 ; . Abscess formation, which consists of acute or chronic or a mixture of both ; inflammatory cells with variable degrees of necrosis. The amount of fibrosis varies during the healing phase. Giant cells are present in some cases only. Diffuse infiltration of histiocytes, in which the infiltrate is composed primarily of histiocytes; few other types of inflammatory cells are present. Only a few well-formed granulomas are seen. Necrosis is universal.
