Pros and Cons of the Liposome Platform in Cancer Drug Targeting Alberto A. Gabizon Shaare Zedek MC and Hebrew University-Faculty of Medicine, Jerusalem Incorporation in the liposome bilayer of a small fraction of polyethyleneglycol PEG ; -derivatized lipids results in inhibition of liposome uptake by the reticulo-endothelial system and significant prolongation of liposome residence time in the blood stream. Parallel developments in drug loading technology have improved the efficiency and stability of drug entrapment in liposomes, particularly with regard to cationic amphiphiles such as anthracyclines. An example of this new generation of liposomes is a formulation of PEG-coated liposomal doxorubicin known as DOXIL, whose clinical pharmacokinetic profile is characterized by slow plasma clearance and small volume of distribution. A hallmark of these long-circulating liposomal drug carriers is their enhanced accumulation in tumors. The mechanism underlying this passive targeting effect is the phenomenon known as enhanced permeability and retention EPR ; which has been described in a broad variety of experimental tumor types. It is still unclear how prevalent is the EPR phenomenon in human cancer, and particularly in metastatic cancer. At a clinical level, DOXIL confers an improved therapeutic index to doxorubicin, but this is mostly accounted for by a reduction of toxicity. Further to the passive targeting effect, the liposome drug delivery platform offers the possibility of grafting tumorspecific ligands on the liposome membrane for active targeting to tumor cells, and potentially intracellular drug delivery. Ligand-specific targeting may enhance tumor drug accumulation and reduce the toxicity of liposomedelivered drugs in comparison to passively targeted systems. The pros and cons of the liposome platform in cancer targeting will be discussed vis--vis non-targeted drugs and drug-ligand conjugates, using as an example a liposome drug delivery system targeted to the folate receptor.

Commercial liposomal drug delivery systems, such as Doxil Caelyx and Myocet, have improved the side effect profile of doxorubicin, particularly by reducing its acute and chronic cardiotoxicity 7, 23 ; . Interestingly, although the doxorubicin release profile of Doxil Caelyx and Myocet are very different, with Doxil Caelyx having very slow release rates in vivo and Myocet fast drug release rates 24 ; , both formulations moderate the cardiotoxicity of doxorubicin compared with conventional doxorubicin, with Doxil Caelyx appearing to have an advantage over Myocet 25.

It affects the metabolism of agents such as doxorubicin and epirubicin, and perturbs cell cycle progression and thereby affects the pharmacodynamics of cell cycle phase specific agents such as gemcitabine and methotrexate Baker and Dorr, 2001; Minotti et al., 2001 ; . The present study showed that paclitaxel also affected the delivery of subsequently administered agents. Furthermore, while an effective paclitaxel tumor priming schedule enhanced the activity without promoting the toxicity of Doxil, a reverse sequence of the combination that disabled tumor priming produced greater toxicity and lethality without producing therapeutic benefits. These complex time- and dose-dependent tumor priming effects of paclitaxel and the multi-dimensional drug-drug interactions highlight the need of careful considerations in using paclitaxel in combination therapy. For the combination of paclitaxel and Doxil, the several schedules under clinical evaluation involve using concurrent administration Briasoulis et al., 2004; Campos et al., 2003 ; . However, in order to ensure the production of cassava mosaic disease CMD ; -free cassava planting materials, in vitro raised cassava plants were first micro-propagated and were then subjected to hardening. Hardened micro-propagated plants were then transplanted under 35% shade net nursery on beds consisting of sand: soil mixture of 50: proportion. Among the spacing adopted in the nursery, 30 x 30 cm was ideal. First cutting from these plants were taken 5 months after planting in the nursery and such stem cuttings were further made in to minisetts of two nodes. A mean of 30 minisetts were obtained from each cutting, and were then planted in a separate nursery under a shade net cover of 35% shade. On attaining one month growth, the minisetts were transplanted to the main field for planting material production. All prescribed agro-techniques required for planting material production was followed and the crop finally harvested on maturity. James H Southard, PhD, is Professor of Surgery at the University of Wisconsin. He joined the university as a post-doctorate in 1970 at the Institute for Enzyme Research. He became manager of the Organ Preservation Research Laboratory, also at the University of Wisconsin, in 1976, developing new methods for long-term organ preservation. The results of that work were a new perfusion preservation solution and a cold storage solution. Dr Southard is past president of the Society for Cryobiology and has been a member of that society since 1985. He is also a member of the American Transplantation Society and the International Transplantation Society. He is on the editorial boards of Cryobiology, Transplantation, and Transplantation International, and he reviews research for many other journals. Dr Southard is also the author of over 250 journal articles and over 30 book chapters, mostly dealing with organ preservation and doxil.
