In this study, the effect of 5-ASA on colorectal carcinogenesis was evaluated in the Swiss Webster model of AOM DSS induced colitis. Female mice age 7 weeks ; were injected with azoxymethane AOM ; 7.4 mg kg i.p. ; and randomized into 4 treatment groups consisting of 25 animals each. Three groups received oral 5-ASA continuously throughout the study at 75, 150 and 225 mg kg body weight. The last group received AOM and DSS alone AOM DSS control ; . 5mM sodium phosphate pH 7 ; served as the vehicle for both 5-ASA and DSS. It should be noted that a dose of 150 mg kg body weight is comparable to the daily dose administered to colitis patients. This notice must include the name of the medication, the amount prescribed, the recommended dosage, the reason for the use of the drug, the efficacy of the medication and its side effects. The child's parent or guardian or their legal representative can file a petition with the juvenile court requesting a hearing to ascertain the appropriateness of the prescription as well as the amount of the drug being prescribed. In addition, the court can order an independent evaluation of the medication and its appropriateness for that child.16 After a child's death from an overdose of prescription antipsychotic drugs, the Washington Department of Social and Health Services DSHS ; formed county-level, 11member review teams to ensure that children on antipsychotic drugs are being cared for properly. In late 1997, the state adopted rules requiring that a foster child cannot be given prescription stimulants, sedatives or anti-depressants without the permission of a biological parent or a judge. In addition, children age 13 years and older that are mentally competent must consent to the administration of their own medication. In 2000, with the passage of the Substitute to House Bill SHB ; 2912, Washington State DSHS instituted a "child health passport" program for foster children. The health passport lists all pertinent medical information regarding the child, and travels with him or her to each placement and each new medical provider. The passport is intended to ensure that physicians serving foster children are informed about each child's previous medical and medication history. SHB 2912 required DSHS to provide the legislature with information on the number of foster children who have used or are using psychotropic drugs.18 This information prompted the 2005 legislature to approve SHB 2985, which created a medical team within DSHS specifically for foster children. The medical team within DSHS helps develop medical policies for foster children, and they help advise the state's Medicaid department on the special needs of foster children.19 According to Washington State DSHS staff, the Medicaid formulary for all children covered by Medicaid, including foster children will be changed. The formulary change, effective September 1, 2006, lists approved psychotropic drugs and includes dosage guidelines for any medication given to a child that is not on the formulary list or not given.

The water at the wreck site proved to be remarkably clear, and although there was one lighting failure the resultant photography was excellent. Currents at the site are typically small, estimated at about a half-knot or less. Net sediment deposition in the sixty years since the sinking appears to be inconsequential, and is often absent entirely, amounting in most cases to a few millimeters of fine unconsolidated mud which was easily in fact usually accidentally -- blown away by the propulsors on the ROV. The edges of the slide scar remain exceedingly sharp and fresh looking, as if they were, literally, made yesterday. Some evidence of previous visits by a MIR expedition team, which visited the site shortly before we arrived, was also seen on the bottom. A memorial plaque had been placed on the after weather deck of Bismarck, and several gouges, identified by our ROV operators as skid marks caused when the MIR exploratory submarines apparently bottomed from time to time, were also observed on the sea-bed. A memorial plaque was placed by this expedition as well. The general condition of the Bismarck wreck is good. A substantial amount of paint adheres to various structures, although as in the case of USS Yorktown corrosion is heavy where fires have removed or damaged this protective coating. As fires were extensive, these corroded areas are extensive as well. The wreck is covered with large numbers of small circular or near-circular corrosion foci ranging from about 20mm to 250mm in diameter. In most cases it is difficult to tell whether these originate from age-related failures of the protective coating due to normal application variances or are due to localized damage due to the impact of shell fragments. "Rustsicle" formation is minimal. No significant pollution arising from fuel oil or any other contaminant was observed, and the marine ecosystem in the vicinity appeared entirely unaffected by the wreck. The main hull itself lies at a depth of approximately 4, 700 meters. A simplified plan of the debris field is given in Figure 9. PURPOSE The purpose of this release is to notify local districts that the mandated DSS-3174: "Recertification Application" and the mandated DSS-3608: "Recertification Guide" have been reformatted, revised and combined into a single document. This document will be available for use by Upstate local districts in November 1992. In addition, the mandated Pub. 1313: "How to Complete . Recertification" has been revised to correspond to the new single document and will also be available in November 1992. II. BACKGROUND The Recertification Application was first implemented in 1979. The Recertification Guide was first developed by an Eligibility Document Redesign Workgroup consisting of State and local district staff which met from 1983 through 1985. The Guide was implemented in August, 1985. In the Spring of 1989, another workgroup of State and local district staff met to revise the eligibility documents. At that time, the Recertification Application was expanded to six pages to mirror the DSS-2921: "Application". In addition, the Recertification Guide underwent a massive overhaul both in format and content. The new format featured a three-way comparison among the case record, the Recertification Application and the interview findings, and it attempted to highlight those areas most likely to change or cause quality control errors. The new format caused a number of problems for local districts. Although the intent was to save examiners time during and after interviews and to reduce errors, it became apparent that, in many cases, completion of the form took more time. In an effort to further the Department's mandate reduction initiatives and, as a result of a suggestion from Monroe County staff, a workgroup of Monroe County and Department staff was formed in July of 1990 to design a combined Recertification Application and Guide. After several meetings in Rochester with Monroe County staff, interviews with examiners and case reviews of recertifications and frequent discussions with program staff from the Department, a Test Recertification Form, along with a revised "How To Complete" Test Publication, was piloted in Monroe County. Wayne County DSS also entered the pilot and, after subsequent meetings with Monroe County DSS and Wayne County DSS staff, along with numerous interviews with examiners working with the documents, a revised version 6 91 ; was printed and again piloted. The revised documents were introduced at NYPWA IM and MA meetings in the Fall of 1991 and the Spring of 1992. Since those meetings, 13 other local districts have also entered the pilot.
